Client Intake Forms

Please arrive 10-15 minutes prior to your first appointment or bring a completed copy of these pages with you.  

Massage intake forms and yoga intake forms must be updated when heath records or medications change or once per year.  Each visit we will take a moment to review and update your forms, as needed.  This is an important part of providing safe, quality care.  Your honesty on these forms is essential to ensure contraindications  do not put you at risk.  These files are stored following HIPAA regulations.
