Client Intake Forms

These forms are specific to special populations (pregnancy, cancer patients, minors) or pertain to specialized treatments (hot stone, wood therapy, cupping) that have additional contraindications or risks.   Please note that minors must be accompanied by a parent or legal guardian.  Please arrive 10-15 minutes prior to your first appointment or bring a completed copy of these pages with you.  

Intake forms must be updated when heath records or medications change or once per year.  Each visit we will take a moment to review and update your forms, as needed.  This is an important part of providing safe, quality care.  Your honesty on these forms is essential to ensure contraindications  do not put you at risk.  These files are stored following HIPAA regulations.
